2. Additional names of all individuals on the deed.
3. Mailing address (if same as property address no need to enter).
4. Primary residence? seasonal residence? (advise how many months it will be occupied).
5. Rental property, or will you be living in it?
6. Mortgage? Escrow Billed?
7. Closing Date? Policy Expiration Date.
8. Any dogs? If so, how many and what breeds?
9. Claims, if any, please provide details.
